Medicare pays for skilled rehab for a limited time, so the practical move is to line up a community before the last covered day, not after. When rehab is ending, ask the discharge planner what help your parent will need, then call a community or two to check openings.

Medicare covers up to 100 days of skilled nursing care in a benefit period, and only after a qualifying 3 day inpatient hospital stay. Many stays end well before day 100, so confirm your parent's last covered day with the rehab team early.
A short stay can be a soft start
Some families are not ready to commit to a full move the week rehab ends. Both communities offer short respite stays, a few days to a few weeks, so a parent can keep recovering in a real apartment while everyone sees how it fits.
